|Consent to Electronic Delivery of Documents Containing Personal Information by the Public Employees Pension
I have read and understand this Consent to Electronic Delivery of Documents and consent to the electronic delivery
of the following documents in accordance with the provisions of this consent:
- PEPP Member Statement; and
- Other documents containing personal information about my pension account.
I understand that the Public Employees Pension Plan will send me notice via email
that the indicated documents have been delivered to the PEPP Access online account information tool and are
available there for me to view.
I acknowledge that I require internet access and registration to the PEPP Access online account information tool to
receive the documents electronically, and I understand how to use PEPP Access to view my confidential personal
information, such as my PEPP Member Statements.
I acknowledge that I may receive a paper copy of the documents at no cost if I contact the Public Employees
Pension Plan via telephone, regular mail, or electronic mail at the contact information listed below.
I understand that the electronic version of the documents will remain available through PEPP Access for a minimum
of three years from the date that it is posted. I also understand that after a three-year period, the documents may be
subject to removal from PEPPAccess, but that I may contact PEPP at any time up to seven years after my PEPP
account is closed to receive a paper copy of the document.
I understand that I will be provided with a paper copy of any documents delivered electronically if electronic
